What to Bring to Your First Appointment
For your first appointment, please bring with you:
- Your family doctor’s name and phone number
- A list of medications, allergies, and medical conditions
- Your health insurance card and booklet, if you have one
LLM Dental Financial Policy
Payment is due when services are rendered unless other arrangements have been made in advance.
If you have dental coverage, we can submit your insurance claim electronically as an "Out of Network Provider" and accept the assignment of benefits. We ask that you keep a credit card on file to cover any portion of the claim not paid. Please remember you are fully responsible for all fees this office charges, regardless of your insurance coverage.
We can make arrangements for a short-term, interest-free payment plan, but you must do this before the actual procedure.
Should you require a long-term payment plan, we will accept CareCredit Extended Monthly Payment Plans (24-60 months).
